If you're a first-time visitor at Nicole Ganz, D.C., you'll need to give us some information about your background, medical history and the nature of your condition. This is so that we can provide the best possible care, tailored to your specific circumstances.

A bit of paperwork

We will need you to fill out forms that provide general information about you and your condition. Our staff is happy to assist you with the paperwork.

 

If you are setting up an appointment over the phone you can ask to have the new patient forms e-mailed to you. This will save you time and enable you to bring the filled out forms with you to your first appointment.

 

Consultation
During your first consultation with the doctor, you'll discuss any health-related issues and concerns. You will also learn about potential treatment options.

 

History and examination
In order to pinpoint the problem, the doctor will ask you a number of questions related to your condition. A variety of targeted tests will be performed to determine which tissues are affected and to what extent.

 

Home instructions
Before leaving, you will be given instructions on certain activities or procedures to be conducted at home. This may include ice applications, avoidance of certain activities or positions, as well as home exercises.

 

Scheduling the next appointment

Regular treatment is essential to the process. We work our treatments around your schedule. Generally speaking, patients are seen within 1-2 days of their initial visit.
